Welcome to our practice! We will make effort to see that your dental experience is as comfortable as possible.
It is the intention of this office to provide for your dental health as thoroughly and as efficiently as possible. We believe to our patients is at its best when there is a complete mutual understanding and cooperation. Therefore, we would like to acquaint you with the customary sequence of procedures for the new patient in our practice.
1. The initial appointment (except in case of emergencies) is spent conducting a thorough examination. This includes a complete health history, necessary x-rays, a visual examination of the teeth, oral cancer screening and gum tissue evaluation. 2. From our x-rays and the clinical finding, we will diagnose and discuss the conditions present in your mouth and recommend proper treatment. Fees for the recommended services are outlined and financial arrangements are made at this appointment. 3. When the method of treatment is decided upon, we will arrange your future appointments so that your dentistry can be completed as efficiently and as quick as possible. 4. Enclosed you will find health information form(s), as well as a patient privacy form (green or blue sheet as two sides). Please bring completed forms to your appointment.
